Il peut \u00eatre cr\u00e9\u00e9 pour tout b\u00e9n\u00e9ficiaire de l&rsquo;Assurance maladie apr\u00e8s recueil de son consentement ou de celui de son repr\u00e9sentant l\u00e9gal. Il peut \u00eatre ouvert par :<\/p>\n<ul>\n<ul>\n<li>les patients ;<\/li>\n<li>tout professionnel de sant\u00e9 ainsi que toute personne exer\u00e7ant sous sa responsabilit\u00e9 ;<\/li>\n<li>les personnes assurant des fonctions d&rsquo;accueil des patients au sein des \u00e9tablissements de sant\u00e9, des laboratoires de biologie m\u00e9dicale, des services sociaux et m\u00e9dico-sociaux ;<\/li>\n<li>les agents de l&rsquo;Assurance maladie.<\/li>\n<\/ul>\n<\/ul>\n<p>Il est accessible aux professionnels de sant\u00e9 par voie \u00e9lectronique notamment depuis un site internet ou via certains logiciels. Il est \u00e9galement accessible \u00e0 son titulaire par voie \u00e9lectronique. Il contient notamment les donn\u00e9es relatives \u00e0\u00a0:<\/p>\n<ul>\n<li>l&rsquo;identit\u00e9 et l&rsquo;identification du titulaire\u00a0;<\/li>\n<\/ul>\n<ul>\n<li>la pr\u00e9vention, l&rsquo;\u00e9tat de sant\u00e9 et le suivi social et m\u00e9dico-social que les professionnels de sant\u00e9 estiment devoir \u00eatre partag\u00e9es (afin de servir la coordination, la qualit\u00e9 et la continuit\u00e9 des soins, y compris en urgence, en particulier l&rsquo;\u00e9tat des vaccinations, les synth\u00e8ses m\u00e9dicales, les comptes rendus de biologie m\u00e9dicale, d&rsquo;examens d&rsquo;imagerie m\u00e9dicale, d&rsquo;actes diagnostiques et th\u00e9rapeutiques, et les traitements prescrits)\u00a0;<\/li>\n<li>la dispensation de m\u00e9dicaments\u00a0;<\/li>\n<li>don d&rsquo;organes ou de tissus\u00a0;<\/li>\n<li>directives anticip\u00e9es\u00a0;<\/li>\n<li>l&rsquo;identit\u00e9 et les coordonn\u00e9es de la personne de confiance\u00a0;<\/li>\n<li>l&rsquo;identit\u00e9 et les coordonn\u00e9es des proches du titulaire \u00e0 pr\u00e9venir en cas d&rsquo;urgence\u00a0;<\/li>\n<li>l&rsquo;identit\u00e9 et les coordonn\u00e9es du m\u00e9decin traitant.<\/li>\n<\/ul>\n<p>Le b\u00e9n\u00e9ficiaire est inform\u00e9 des finalit\u00e9s du DMP ainsi que de ses modalit\u00e9s de cr\u00e9ation, de cl\u00f4ture et de destruction.
